The details of your current Blue Badge (if you’re reapplying)
Important: Previous badges that expired more than one month ago cannot be submitted as a renewal.
If you are eligible to apply you will need:
- your National Insurance number (if you have one)
a photo or scan of your:
- photograph of who the badge is for
- proof of benefits (if you get any)
- e-visa, Birth Certificate (if no national insurance number)
- NHS number
As part of the application process we ask for your NHS number which enables us to access key health records to gather evidence on your behalf and will assist us to process an application quicker, consent to access your health records gives permission to the Local Authority to both engage with you to carry out an assessment and to gather relevant information from organisations or other people who can assist with the assessment.
If you are unable to or do not wish to provide your NHS number, this may result in an inability to conclude your assessment in a timely way.
Information collected is stored in accordance with Sunderland City Council's Retention Schedule for Adult Social Care and will only be kept for as long as required. Information no longer required will be destroyed securely by deletion of electronic records
A Blue Badge costs £10 and usually lasts up to 3 years.
